Placement in the desired field is not always possible, so fill in more than one choice. If you fill in only one choice, this means that you agree to any department if the department of choice is not possible. SloMSA participates on Academic Quality project, so we recommed you to bring your handbooks. We will not accept any AFs where a student has not studied any of the fields he/she has chosen. Student has to choose at least 2 fields that he/she has already studied AND in which he/she has passed his/her exams. |

| Application documents & deadline | All application documents MUST be sent by e-mail to neo_in_slomsa@yahoo.com at LEAST THREE months before the clerkship starts or given to NEO IN on CD on MM. All documents must be OFFICIALLY translated into ENGLISH, and have all necessary Medical Faculty, and IFMSA stamps where appropriate. |

| Insurance | Proof of Health Insurance is needed. Travel Insurance is also recommended. Malpractice insurance is not required. |

| Board & Lodging | Board-1time per week days. Lodging-provided by Dormitories and Student Hostels. Lodging and boarding -free for bilaterals, for unilaterals for 270 euro. |
| Social Program | Provided by Local Committees, available in all towns.
There is a welcome party, tours around city and surroundings, trips to the most attractive areas of Slovakia (High Tatras, Slovak Paradise, historical villages, castles, caves…), sightseeing, visiting museums, galleries, visiting pubs ... All contact persons are volunteers and they are not payed for the job they are doing. Please, keep it in your mind. | Substitutions | Must be from the same sex, the same city, period and department.
Must be reported to the NEO 4 weeks before the beginning of the clerkship. |
| Cancellations | We prefer substitutions to cancellations, but if they have to be made, we need to know 4 weeks in advance. Please contact NEO IN SloMSA. |
| Visas | Every student is responsible for getting the visa. Please apply for tourist visa. Invitation letter IL will be send to NEO and then distributed to students. Contact the NEO-In SloMSA if you need any other necessary documents. Some countries (especially african) need to have special IL with police stamp. This stamp costs 35 euro + 30 euro other administration fees which has to pay student after arrival to LC. It is necessary to send all the documents at least 3 months before arrival, because procedure in police takes more than month. For more information please contact NEO IN SloMSA |

No student will have board and lodging outside the dates specified in the Card of Acceptance. Exchange students work under the same conditions as Slovak students,and must obey all regulations at the hospital. If the exchange student does not,he/she will be expelled from the clerkship.We expect the exchange student to participate seriously and actively at the department. Student will get certificate if attends department more than 3 weeks. Clerkships may be automatically cancelled when the Local Committee receives claims from the hospital or the dormitories. Every student should fill in Incoming Evaluation form after finishing the clerkship, but before leaving the Slovakia. This UNI fee must be send to SloMSA account at least till the end of the march. If the student cancels clerkship 1 month before arrival will get all the UNI fee back, if 2 weeks before arrival, 150 euros, if later no fee will be returned. Students have to pay deposit of 25EURO - 100 euro/ varies from city. It will be returned at the end of clerkship after filling the Evaluation Form. |
